Response to commentary re 'Reduction and Standardization of Surgical Instruments in Pediatric Inguinal Hernia Repair'

Continuous process improvement, as the name implies, is a changing and ongoing process in the quest for positive gain in a system. The analogy is like in baseball: is a batter doing anything to keep the inning alive with the hope of the ‘team’ scoring the necessary winning run, knowing that hitting the walk-off home run is not always achievable? Stakeholder engagement and, importantly, executive/senior buy-in are the keys to implementing change, and to sustaining and promoting further change. To change a culture, abrupt and oft en-changing management philosophies can actually ‘disengage’ stakeholders.
