Multiparameter Investigation of a 46,XX/46,XY Tetragametic Chimeric Phenotypical Male Patient with Bilateral Scrotal Ovotestes and Ovulatory Activity

We report on an adult male initially presenting with gynecomastia and a painless scrotal mass without additional genital anomalies. Hyperpigmentation of the skin following the Blaschko's lines was identified. He underwent gonadectomy because of suspected cancer. Histological analyses revealed an ovotestis with ovulatory activity confirmed by immunohistochemistry with multiple markers. Karyotyping of cultured peripheral blood lymphocytes and a buccal smear revealed a 46,XX/46,XY chimeric constitution with different percentages. Multiple molecular analyses as well as blood typing implied a tetragametic origin. After the unilateral gonadectomy, the patient developed recurrent painful cystic swellings of the remaining gonad. Because of the wish to preserve hormonal activity as well as future fertility, the patient underwent surgical resection of a cystic gonadal area. The removed tissue showed ovulation-related features in addition to both testicular and ovarian tissue, diagnosed as an ovotestis. Testosterone therapy was initiated to suppress the persistently elevated gonadotropins and thereby suppress ovarian activity. During treatment, the recurrent pain complaints and cystic swellings ceased, although gonadotropin levels were not fully suppressed. Based on these observations, the importance of a detailed genetic and pathological diagnosis and the clinical dilemmas including the pros and cons of personalized treatment with gonadal preservative surgery are discussed.Sex Dev

Accumulating convincing evidences show that exercise reduces cancer progression and recurrence mainly in colon and breast cancer patients. Interestingly, the positive effects of exercise on cancer outcomes were mainly observed when patients practiced vigorous exercise of 6 METs or more. At the molecular ...

Authors: Bharadwaj AS, Swamy PM, Mamas MA Abstract Introduction: There are numerous shared risk factors in the etiopathogenesis of coronary artery disease (CAD) and cancer both at the epidemiologic and molecular level. Additionally, the different modalities of treatment of cancer such as radiation, chemotherapy, immunotherapy and hormonal therapies further increase the risk of CAD and acute coronary syndrome. Most large database analysis and single center experiences have shown that cancer patients undergoing PCI are at an increased risk of in-hospital mortality, bleeding repeat revascularization.Areas covered: In ...

Authors: Annibale B, Esposito G, Lahner E Abstract SUMMARYIntroduction: Atrophic gastritis (AG) is a complex syndrome which arise as consequence of H. pylori infection or in the context of gastric autoimmunity. It often deserves a benign course, but may lead to potentially life-threatening complications: cancer and anaemia. This review aims to address traditional and innovative knowledge on this often under-diagnosed disorder.Areas covered: This review covers clinical presentation, risk factors, diagnosis and management of AG and provides an updated resource for clinicians to get insight in this challenging disorde...

This article seeks to shed light on the dualistic role of Wnt signalling in liver regeneration following injury and how Wnt signalling can regulate scar formation. It also discusses how Wnt signalling cooperates with other classical fibrogenic signalling cascades, such as TGFβ signalling. Finally, the article examines recent advances in the development of Wnt signalling pathway inhibitors and asks whether repurposing these agents as anti-fibrotic therapies is a realistic option.Expert Opinion: The understanding of Wnt signalling in liver regeneration and fibrosis is in its infancy and whilst new generations of Wnt pat...

Abstract Protein N-glycosylation plays crucial roles in many biological processes and has close association with the occurrence and development of various cancers. Therefore, it is necessary to analyze the abnormal changes of N-glycopeptides in complex biological samples for biomarker discovery. However, due to their low abundance and poor ionization, N-glycopeptides identification in complex samples by mass spectrometry (MS) is still a challenging task. In this work, a novel magnetic hydrophilic material was prepared by serial functionalization of ultra-thin two-dimensional molybdenum disulfide with Fe3O4 nanopar...

Muffin tops, man boobs, and bagel bumps: These are among the varied and perverse ways that the hormonal distortions inflicted on unwitting humans who consume the seeds of grasses, i.e., grains, show themselves. In our modern world filled with thousands of processed foods, there are plenty of landmines for health. Gummy bears and gumdrops will rot teeth, for instance. Indulge in a handful of dried prunes and you’ll have to schedule a substantial portion of your day on the toilet due to bowel irritants. But only wheat and grains are associated with a wide swath of health problems that range from autoimmune disease to m...
