The Value of Echocardiography in Unexpected Valve Disease in a Patient With Ischemic Cardiomyopathy: Less Is Not Always the Right Answer

A 72-YEAR-OLD MALE with an unremarkable medical history presented to an outside hospital with progressively worsening lower extremity edema and dyspnea on exertion. At the outside hospital, he underwent transthoracic echocardiography and left-heart catheterization, which revealed depressed left ventricular function, with an estimated ejection fraction of 30% to 35%, two-vessel coronary disease, and severe mitral regurgitation (MR). Preoperative laboratory evaluation also was notable for stage-III chronic kidney disease and mild anemia.
