Role of Mesh in Component Separation: A Meta-Analysis

Ventral incisional hernias treated with primary closure or bridging meshes are associated with high recurrence rates approaching 50-60%. The component separation technique was devised to provide a tension-free repair by mobilization of native tissue. The original description by Ramirez and colleagues did not include placement of mesh; however, modified techniques include the addition of mesh. The rationale for mesh insertion has been based on small, observational data. We predict that the adjunctive use of mesh with component separation will reduce recurrence rates.
