138 Failure of timely referral to acute in-hospital heart failure services

Introduction: Acute heart failure is the leading cause of hospital admissions inpatients 65 years or older. Current guidelines from National institute for Health and Clinical Excellence (UK) recommends all patients admitted with acute heart failure be seen within 24  hours of admission by a specialist multidisciplinary team. Early input from a dedicated heart failure team reduces mortality and hospital readmission rates. The ESC advices appropriate referral and consultation with the multidisciplinary team. We evaluated adherence to these guidelines at our in stitution.
