Despite the self-limiting nature of lateral epicondylitis (LE) the disease has a prolonged course in some patients. Currently there is no optimal treatment of choice for these patients. We treated 6 elbows in 5 patients with LE with free fat grafting. All patients had symptomatic LE (symptom duration>2 y) and magnetic resonance imaging scan consistent with LE, and no finding of osteoarthrosis of the elbow joint, ligament injury, or other condition causing palpable lateral elbow pain. The fat graft was harvested from the abdomen, centrifuged or decanted and 10 mL was injected in and around the lateral epico...

Ask a smoker what they get out of cigarettes and they are likely to talk about pleasure, contentment, and an overall good feeling. Nicotine, the active ingredient in cigarettes, is a stimulant. Used in low doses like those delivered by combustible cigarettes, stimulants activate the nervous system, resulting in enhanced arousal and alertness. Nicotine binding in the limbic system — the part of the brain that houses the pleasure and reward center — releases dopamine, resulting in feelings of euphoria. These effects combine to give smokers a boost in their mood. In this context, new research from a team at Harvar...

We examined the effect of pre-operative narcotic use in addition to other predictors of the pattern and duration of post-operative narcotic usage.MethodsWe conducted a retrospective cohort study at a single academic institution of patients undergoing a one- or two-level primary TLIF between 2014 and 2017. Total oral morphine milligram equivalents (MMEs) for inpatient use were calculated and used as the common unit of comparison.ResultsA multivariate binary logistic regression (R2 = 0.547, specificity 95%, sensitivity 58%) demonstrated that a psychiatric or chronic pain diagnosis (OR 3.95,p = 0.0...

Behavioral alterations, like mechanical and thermal hyperalgesia, and modulation of biomarkers in the peripheral and central nervous systems (CNS) are markers of chronic pain. Transcranial direct current stimulation (tDCS) with exercise is a promising therapy for pain due to its neuromodulatory capacity.

Initially intended for the treatment of acute or terminal pain, opioids are now utilized in the treatment of chronic nonterminal pain [1 –3]. This has been one of the contributors to increased opioid medication use and abuse within the United States [2,4]. Due in part to the emphasis on adequate pain treatment, the United States consumes 80% of the world's opioid medication [1,2,5].

It’s been a while since I last posted on my blog, as I’ve had other projects on the go this summer. Over this time I’ve been pondering, as I usually do, why pain management/rehabilitation has so many problems. Conceptually, I can understand that pain is a complex experience that we’re a long way from understanding. I get that it’s a philosophically challenging subject. That because it’s subjective (like love, disgust, fatigue or hunger) it’s difficult to examine dispassionately. I also get that it’s big business. Pain is one aspect of being human that captures the entrepr...

Authors: Abstract Chronic pelvic pain is a common, burdensome, and costly condition that disproportionately affects women. Diagnosis and initial management of chronic pelvic pain in women are within the scope of practice of specialists in obstetrics and gynecology. The challenging complexity of chronic pelvic pain care can be addressed by increased visit time using appropriate coding modifiers, as well as identification of multidisciplinary team members within the practice or by facilitated referral. This Practice Bulletin addresses the diagnosis and management of chronic pelvic pain that is not completely explai...
