A Case of Worsening Dyspnea and Cough

Bob is a 72-year-old male, with a 40-pack-year smoking history. He quit about 8 years ago, when severe COPD was diagnosed. Until last year, he had been stable on his regimen of Advair 250/50 and Spiriva Handihaler, other than an occasional “bronchitis,” usually treated with an antibiotic and/or steroids. Over the past year or so, however, he has noticed a marked turn for the worse. He no longer can walk up the steps in his house without stopping and gasping for breath.
