This patient has compound issues, including irregular astigmatism, central corneal scarring, and anisometropia. These issues were induced by PRK performed on the LASIK flap with subsequent healing issues. This is a highly atypical response to PRK after LASIK,1 and it is challenging to determine the underlying etiology for this healing response. The appearance is not suggestive of herpes simplex virus (HSV); however, HSV would complicate management because further excimer laser ablation would not be warranted.
