Acute respiratory distress revealing massive pulmonary embolism in a neonate

A male neonate was delivered at 38 weeks of gestation. The pregnancy and delivery were uneventful. At 15 days of age, at home, he suddenly presented respiratory difficulties and cyanosis. At the arrival in the ICU, the baby was lethargic, had respiratory distress and generalized cyanosis. Oxygen saturation was 70% on 100% fraction of inspired O2 with facial mask ventilation. He required endotracheal intubation for respiratory support. Chest X-ray was unremarkable. Electrocardiogram revealed sinus tachycardia and right ventricular hypertrophy.
