Syncope and paroxysmal atrioventricular block

Publication date: Available online 8 May 2017 Source:Journal of Arrhythmia Author(s): Milena Aste, Michele Brignole Current literature reveals three types of paroxysmal atrioventricular block (AVB) that can cause syncope: Intrinsic paroxysmal atrioventricular block is due to an intrinsic disease of the AV conduction system; this type of “cardiac syncope” is also called Stokes-Adams attack; Extrinsic vagal paroxysmal atrioventricular block is linked to the effect of the parasympathetic nervous system on cardiac conduction and is one of the mechanisms involved in “reflex syncope.” Extrinsic idiopathic paroxysmal atrioventricular block is associated with low levels of endogenous adenosine and is supposed to be one of the mechanisms involved in “low-adenosine syncope.” These three types of paroxysmal AVB present different clinical and electrocardiographic features. Additionally, the efficacy of cardiac pacing and theophylline therapy to prevent syncopal recurrences is also different for these three types of AVB.

Conclusions: This study suggests that neither obesity nor extensive sports seems to result in pathological cardiac repolarization parameters in healthy children. Therefore, pathology has to be assumed if abnormal repolarization parameters are seen and might not be simply attributed to the child's habitus or an excellent level of fitness.

ConclusionLoperamide overdose is a toxidrome that remains underrecognized, and in patients with unexplained cardiac arrhythmias, loperamide toxicity should be suspected. We report a patient with severe VDD that resulted in TdP and cardiac arrest. Post-resuscitation work up illustrated prolonged QTc interval of 620 ms, significant hypocalcemia, and severe VDD of 4 (normal 30–80) ng/mL. After high dose vitamin D/calcium supplements, repeat electrocardiogram revealed normal QTc interval of 423 ms. During hospitalization, the patient suffered no additional arrhythmias and QTc continued to be normal.
