Sharing Mayo Clinic: Persistence propels patient through double-lung transplant
Steve Shank knows what it's like to face an uphill climb. Despite being legally blind and affected by albinism, he has been an avid bicyclist for years, competing in 100-mile rides on the rolling hills of Iowa. The arduous contests challenged Steve physically and mentally, giving the Iowa City native an ability to endure difficult [...]
AS A former fitness fanatic it is typical of Ron Flewett to approach illness with the same energy and vigour with which he has always embraced life. It is all the more impressive for the fact that Ron suffers from a debilitating and incurable lung condition called idiopathic pulmonary fibrosis .

This study prospectively evaluated outcomes of a new approach for these lungs in our lung transplant program.MethodsFrom 2/2016—12/2018, 336 lung transplants were performed at Cleveland Clinic. Of these, 58 met criteria for our aggressive approach to donors with P/F ratio
CONCLUSION: In a porcine DCD model of 9 hours CSP followed by four hours of EVLP the use of modified Custodiol-N as perfusion solution was feasible and associated with higher oxygen capacity than STEEN SolutionTM. The addition of albumin seems to further stabilize lung function. PMID: 32051744 [PubMed]

This study aimed to re-examine the hypothesis that airways are spared in IPF using a multiresolution imaging protocol that combines multidetector CT (MDCT), with micro-CT and histology.MethodsThis was a retrospective cohort study comparing explanted lungs from patients with severe IPF treated by lung transplantation with a cohort of unused donor (control) lungs. The donor control lungs had no known lung disease, comorbidities, or structural lung injury, and were deemed appropriate for transplantation on review of the clinical files. The diagnosis of IPF in the lungs from patients was established by a multidisciplinary cons...
ConclusionThe incidence of de novo malignancy was 1% at a median follow up of 42 (32-73) months. De novo malignancies following LDLT, although uncommon, are associated with significant mortality. A careful screening protocol should be followed post transplantation for early detection of de novo malignancies.
AbstractAlthough short- and medium-term outcomes after liver transplantation for alcohol-related liver disease (ARLD) are generally excellent and similar to outcomes for transplantation for other indications, a return to alcohol consumption commonly occurs even though rates of alcohol consumption after transplantation for ARLD are comparable to those seen in other indications. Transplant recipients should be questioned about alcohol use post-transplantation and, where appropriate, monitored; those drinking significant amounts should be offered treatment with the help of a multi-disciplinary team. Although short-term signif...
