Conclusions: EUS drainage of WON using specially designed FCSEMS is safe and efficacious in children. The utility of FCSEMS in children should be further explored and compared with plastic stents.

For biliary drainage in patients with cancer of the ampulla of the Vater (AV), selective bile duct cannulation is often difficult due to tumors on the orifice of the AV. Bleeding after endoscopic sphincterotomy on the tumor and post-endoscopic retrograde cholangiopancreatography pancreatitis (PEP) should be prevented. Endoscopic ultrasound (EUS)-guided biliary drainage is not recommended for Preoperative biliary drainage in these patients. We easily achieved successful biliary drainage by endoscopic fistulotomy in a patient with a dilated bile duct causing oral AV protrusion after unsuccessful transpapillary bile duct cannulation.

We report this case of a rare complication of chronic pancreatitis to show that color-Doppler ultrasound is a non-invasive and effective diagnostic tool for pseudoaneurysm, which enables early detection and prompt treatment without the need for invasive diagnostic modalities.

Conclusions: EUS-PD was an appropriate treatment for not only benign strictures but also malignant strictures with recurrent pancreatitis after failed ERP.
