Echo drop out in interatrial septum

An echo drop out in the interatrial septum is often noted in the apical four chamber view. This need not mean that there is an atrial septal defect. This is because the thin atrial septum in the region of the foramen ovale is often not imaged well in the apical four chamber view as the ultrasound beam is parallel to the inter atrial septum. For a good ultrasound image, the structure being imaged should be perpendicular to the ultrasound beam. In case of Doppler the beam should be parallel to the flow being imaged. If colour Doppler flow mapping is used, the flow across the atrial septal defect can be seen well in the apical four chamber view, as the flow travels from the left atrium to the right atrium if there is a true atrial septal defect. In this case colour flow mapping did not show a flow across the atrial septum. Subcostal four chamber view is better for looking at atrial septal defects because the ultrasound beam is perpendicular to the atrial septum in that view. CONCLUSION:  CCH can be diagnosed prenatally by detailed fetal echocardiography when observing an abnormal four-chamber view with crossing of inflow streams into both ventricles and a lack of parallel orientation of the atrioventricular valve axis due to a clockwise or counterclockwise rotation of the ventricular mass along its axis. A 1-day-old girl born with a thin collodion membrane was found to have severe thrombocytopenia of 34,000 per microliter. The rare report of the perinatal-lethal subtype of Gaucher disease presenting as a collodion baby represented an explanation for both signs and we recommended early genetics investigation. In the ensuing week, NICU evaluation revealed fevers, bradycardia, hypotonia; labs showed transaminitis, hypoglycemia requiring a continuous glucose infusion; ultrasound discovered hepatosplenomegaly, mild ascites, echogenic kidneys, scoliosis, an atrial septal defect and dilated right ventricle.
