For primary breast augmentation, which is the best approach to implant size selection?Plastic and Reconstructive Surgery
This article discusses the history of breast implants since their invention in 1962. Particular emphasis is given to the evolution of silicone implants with its many challenges, which has resulted in the development of the currently used fourth- and fifth-generation devices. Knowledge of these advances will allow physicians to more critically evaluate their results, and also will encourage them to provide more up-to-date scientific data on these devices to further improve the clinical outcomes of their patients. Seminars in Plastic Surgery 2019; 33: 240-246 DOI: 10.1055/s-0039-1696963One of the most influential factors in the success of breast reconstruction is whether or not radiation therapy has or will be performed. While traditional teaching is that all breasts treated with radiation therapy must be reconstructed with an autologous component, many reconstructive surgeons perform implant-based breast reconstruction without an autologous component and have success doing so. The purpose of this article is to explore the risks, benefits, and nuances of performing implant-based breast reconstruction in the setting of radiation ther...
Seminars in Plastic Surgery 2019; 33: 270-278 DOI: 10.1055/s-0039-1696998Breast implant-associated anaplastic large cell lymphoma (BIA-ALCL) is an emerging and indolent, but potentially fatal cancer of the immune system that can develop around textured-surface breast implants. The World Health Organization first recognized BIA-ALCL as a unique clinical entity in 2016. To date, over 600 confirmed cases have been reported worldwide. BIA-ALCL most commonly presents with disease confined to the capsule, as a seroma or a mass adjacent to the implant. While BIA-ALCL has a fairly indolent clinical course, with an excellent progno...

ConclusionsThe polyurethane surface should not be considered textured in the clinical point of view. Previous experience with non-PU implants cannot be transferred to PU implants. The learning curve is unavoidable. The systematization of errors with PU implants facilitates a decision-making process during the primary and secondary surgery and lowers the risk of the unsatisfactory results.Level of Evidence IVThis journal requires that authors assign a level of evidence to each article. Both Real Housewives of Beverly Hills star Eden Sassoon and former supermodel Angie Everhart told The Doctors the believe their breast implants left them sick and in pain for many years.
Breast augmentation is the second most commonly performed aesthetic surgery in the United States and the use of different implant materials has changed over the years. Aesthetic goals and results in implant-based breast surgery vary widely based on age, geography, and surgeon preference.1 Most surgeons have emphatically returned to silicone implants once the moratorium on them was lifted, claiming they yield identifiably better results. This is seen in the United States, where more than two-thirds of breast augmentations use silicone implants.
