Accuracy of coronary computed tomography angiography for bioresorbable scaffold luminal investigation: a comparison with optical coherence tomography

AbstractTo establish the accuracy of coronary computed tomography angiography (CTA) for in-scaffold quantitative evaluation with optical coherence tomography (OCT) as a reference. The translucent backbone of the bioresorbable scaffold allow us to evaluate non-invasively the coronary lumen with coronary CTA. In the ABSORB first-in-man studies, coronary CTA was shown to be feasible for quantitative luminal assessment. Nevertheless, a comparison with an intravascular modality with higher resolution has never been performed. In the ABSORB Cohort B trial, 101 patient with non-complex lesions were treated with the fully biodegradable vascular scaffold. For this analysis, all patients who underwent coronary CTA at 18 months and OCT within ±180 days were included. Coronary CTA and OCT data were analysed at an independent core laboratory for quantitative cross-sectional luminal dimensions. The primary objective was the accuracy and precision of coronary CTA for in-scaffold minimal lumen area assessment, with OCT as a reference. Among the 101 patients of the ABSORB Cohort B trial, 35 underwent both OCT and coronary CTA. The feasibility of quantitative evaluation was 74%. Recent retrospective studies from Wuhan, China suggest Novel Coronavirus Disease 2019 (COVID-19) may be associated with a hypercoagulable state and increased risk for venous thromboembolism. The overlap in the signs and symptoms of COVID-19 associated Acute Respiratory Distress Syndrome (ARDS) and COVID-19 with concurrent pulmonary embolism creates a diagnostic challenge for emergency medicine physicians in patients already at risk for renal impairment. However, identifying features atypical for COVID-19 alone may play a role in the judicious use of Computed Tomography Angiography among these patients.

Identifying coronary artery disease (CAD) in atrial fibrillation (AF) patients improves risk stratification and defines clinical management. However, the value of screening for subclinical CAD with cardiac CT in AF patients is unknown. Between 2011 and 2015 94 consecutive patients without known or suspected CAD (66 (57 –73) years, 68% male), who were referred for AF evaluation, underwent a non-contrast-enhanced coronary calcium scan and a coronary computed tomography angiography (CCTA) at our center.

Patients with neurofibromatosis type 1 (NF1) can develop both benign and malignant tumors throughout their lives. A 49-year-old man was transferred to the emergency department with complaints of sudden right dorsal pain and respiratory discomfort. He was in shock on arrival. On finding significantly decreased permeability of the left lung field in chest X-ray, drainage was immediately performed. Subsequent computed tomography (CT; Lammert et al., 2005) angiography revealed the extravasation of contrast media from the deep carotid artery, a branch of subclavian artery.

Contrast induced nephropathy is a possible adverse event in fenestrated endografting (FEVAR). Automated CO2-angiography has been proposed as an alternative to iodinated contrast media (ICM) for standard endovascular aneurysm repair; however, its use in FEVAR has not been investigated yet. The aim of this study was to analyze the possibility of reducing the amount of procedural ICM during FEVAR by combining CO2 with intraprocedural 3D preoperative computed tomography angiography overlaid onto 2D live fluoroscopic images (fusion imaging - FI).

We describe the case of a 39-year-old male, who presented to our emergency department after a one-day history of headache and vomiting, with associated sudden onset posterior neck pain and cerebellar signs following a massage.
