Abstract
During the three-year Centers for Medicare & Medicaid Services Federally Qualified Health Center (FQHC) Advanced Primary Care Practice (APCP) Demonstration, 393 FQHCs (78.6% of the original 500 and 90.6% of the 434 continually-enrolled FQHCs) achieved patient-centered medical home (PCMH) recognition. Facilitators included survey fee coverage and technical assistance.
