Pulmonary manifestations in collagen vascular diseases.

[Pulmonary manifestations in collagen vascular diseases]. Radiologe. 2016 Sep 22; Authors: Vogel MN, Kreuter M, Kauczor HU, Heußel CP Abstract CLINICAL/METHODICAL ISSUE: Pulmonary complications are frequent in patients with collagen vascular diseases (CVD). Frequent causes are a direct manifestation of the underlying disease, side effects of specific medications and lung infections. STANDARD RADIOLOGICAL METHODS: The standard radiological procedure for the work-up of pulmonary pathologies in patients with CVD is multidetector computed tomography (MDCT) with thin-slice high-resolution reconstruction. PERFORMANCE: The accuracy of thin-slice CT for the identification of particular disease patterns is very high. The pattern of usual interstitial pneumonia (UIP) representing the direct pulmonary manifestation of rheumatoid arthritis (RA) can be identified with a sensitivity of 45 % and a specificity of 96 %. ACHIEVEMENTS: Both direct pulmonary manifestations, drug-induced toxicity and certain infections can have a similar appearance in thin-slice MDCT in various forms of CVD. Knowledge of the patterns and causes contributes to the diagnostic certainty. PRACTICAL RECOMMENDATIONS: At first diagnosis of a CVD and associated pulmonary symptoms thin-slice MDCT is recommended. Clinical, lung function and imaging follow-up examinations should be performed every 6-12 months depending on the results of the MDCT. In every ca...

ConclusionsIPAF seems to a distinct entity, with a low tendency to evolve in a definite CTD. Nevertheless, further studies are needed to better define the clinical evolution and the outcome of IPAF.

Objective: to identify risk factors for interstitial lung disease (ILD) in patients with rheumatoid arthritis (RA).Material and Methods: The study included 167 patients, 73.1%- women, average age -52 (39; 60) years. A diagnosis of rheumatoid arthritis was made according to the American College of Rheumatology/ European League Against Rheumatism criteria by specialist rheumatologists, with an average number of points for 4 positions - 8.4±3.11. Median DAS28 was 4.8 (3.3; 6.4) points, positive for rheumatoid factor (RF) – 49 (73.1%). Anti-citrullinated protein antibodies (ACPAs) were detected in 45 (67.2%). ILD ...

An international consensus for rheumatoid arthritis (RA) patients at risk of developing interstitial lung disease (ILD) is still lacking. The aims of study were to evaluate: the prevalence of ILD involvement in RA over high-resolution computed tomography (HRCT); the relationships between pulmonary function tests (PFTs), patient-centered measurements, and ILD; and the potential risk factors contributing to RA-ILD patients. Data regarding the clinical characteristics (age, sex, age at onset of RA), laboratory findings (rheumatoid factor [RF] and anti-citrullinated protein antibodies [ACPA]), respiratory functional assessmen...

Interstitial lung disease (ILD) is a severe systemic manifestation of rheumatoid arthritis (RA). High-resolution computed tomography (HRCT) represents the gold standard for the diagnosis of ILD, but its routin...

In this report, the clinical relevance of plasma Wnt5a protein was evaluated in 40 RA-ILD patients and 41 non-ILD RA cohorts. The results showed an elevated Wnt5a protein in plasmas of RA-ILD patients compared with non-ILD RA patients (p

Authors: Fukada A, Toyoshima M, Nozue T, Suda T Abstract An 81-year-old woman with rheumatoid arthritis (RA) who had been treated with bucillamine presented with dyspnea. Computed tomography of the chest showed ground-glass opacities and consolidations in both lungs and honeycombing in both basal lung areas. An elevation of the serum Krebs von den Lungen-6 level and hypoxemia were seen. Lymphocytosis with a decreased CD4/CD8 ratio was seen in the bronchoalveolar lavage fluid. A transbronchial lung biopsy specimen showed organizing pneumonia. Based on a diagnosis of bucillamine-induced pneumonitis (BIP) with RA-asso...

Publication date: Available online 8 March 2019Source: Respiratory InvestigationAuthor(s): Tomoo KishabaAbstractIdiopathic Pulmonary Fibrosis (IPF) is a most common progressive interstitial lung disease (ILD) of unknown etiology, although majority of patients are elderly male smokers. The main pathogenesis is aberrant recovery of epithelial injury and collagen deposition. Fibrotic nonspecific interstitial pneumonia, connective tissue disease (CTD) especially rheumatoid arthritis (RA) associated ILD, and chronic hypersensitivity pneumonia(CHP) are important differential diagnosis. Main symptoms are non-productive cough and ...

ConclusionsThese results suggest that NSIP and bronchiolitis HRCT patterns may be risk factors for resistance to RA therapy.

New research has linked the risk of developing interstitial lung disease in rheumatoid arthritis patients with the promoter variant in MUC5B, which may also contribute to the development of idiopathic pulmonary fibrosis and an unsual interstitial pneumonia seen by high-resolution CT scan...
