BRCA1 mutations linked to increased risk of serous, serous-like endometrial cancer

(The JAMA Network Journals) Increased risk for aggressive serous/serous-like endometrial cancer was increased in women with BRCA1 mutations, although the overall risk for uterine cancer after risk-reducing salpingo-oophorectomy (RRSO) to remove the fallopian tube and ovary was not increased, according to a new study published online by JAMA Oncology.

Conclusion This is a simple and reproducible technique for preventing major complications associated with LESS salpingectomy. This approach permits easier specimen retrieval because of the large solitary incision that is made. Conclusion Gynecologic surgeons often perform castration oophorectomy in breast cancer patients as more oncologists adopt aromatase inhibitors for treatment of premenopausal breast cancers. Our data suggests other abdominal/pelvic cancers, precancerous conditions, and previously unrecognized metastatic disease are not uncommon findings in this patient population. The gynecologist serving this patient population may consider careful abdominal survey, pelvic washings, endometrial sampling, and request serial sectioning of fallopian tube specimens for a thorough evaluation.

Conclusions: With longer follow-up, our rates of local recurrence following breast-conserving therapy for DCIS remain very low (1.5% at 10 years). The vast majority of patients had>2 mm margins, specimen radiographs, and received a tumor bed boost. The majority (60%) of patients with hormone receptor positive disease received adjuvant endocrine therapy. The 10-year incidence of CBC was higher than expected. Conclusions. Patients with BRCA1 gene mutations have relatively low serum HE4 levels. Even the slightest elevation in HE4 or CA 125 levels in female BRCA1 carriers undergoing prophylactic surgery should significantly increase oncological alertness.
