Microbiology of Middle Ear Infections: Do You Hear What I Hear?

Infectious conditions of the middle ear are a common and significant cause of morbidity and sometimes even mortality, especially in young children and elderly individuals. Pathogens and harmless commensal bacteria, viruses, and fungi co-inhabit the auditory canal and form intricate ecological networks, collectively known as a microbiome. Few studies that describe the normal flora of the middle ear have been published, and controversy exists about the roles of several possible pathogens. This review describes current literature examining otitis media and the roles various microbes play in the pathogenesis of middle ear infections.

Conclusion: While the exact role of intratympanic injections requires additional trials, triamcinolone resulted in similar outcomes compared to studies using dexamethasone or methylprednisolone. Due to favorable pharmacological properties, triamcinolone represents a safe and efficacious alternative for intratympanic treatment in idiopathic sensorineural hearing loss.

Conclusions: Data from the current study suggest that canalplasty offers benefit regarding closure rate in medial graft tympanoplasty. Use of cartilage also portended a higher rate of perforation closure. Canalplasty should be considered when using medial graft techniques if exposure is limited due to bony canal anatomy.

Conclusion: Neglected mastoid cavity care, low socioeconomic status, poor personal hygiene, homelessness, and chronic suppurative otitis media are likely contributing factors to this uncommon presentation occurring in an urban setting.

DiGeorge syndrome (DGS) is a primary immunodeficiency characterized by various degrees of T-cell deficiency. In partial DGS (pDGS), other risk factors could predispose to recurrent infections, autoimmunity, and allergy. The aim of this study was to assess the effect of different factors in the development of infections, autoimmunity, and/or allergy in patients with pDGS. We report a rare case of a nine-month-old infant who showed right temporal mass with no evidence of infection. Computed tomography showed an osteolytic round mass, and magnetic resonance imaging revealed heterogenous enhancement with a high apparent diffusion coefficient. Malignant tumor was first suspected, but an open biopsy revealed the swelling to be temporal muscle abscess. It should be noted that temporal abscess may mimic the features of a malignant tumor, and multiple examinations should be performed for an accurate diagnosis.
