SUMMARY ANSWER: Multifaceted safety initiatives were implemented at the main academic center and across all network sites at the University of Pennsylvania. A comparison of preintervention and postintervention data demonstrates less frequent SRMEs from 2009-2016 (Fig 1). The Agency for Healthcare Research and Quality (AHRQ) results demonstrate high patient safety culture grades among staff members over time. We are confident that the collective implementation of the following safety initiatives has led to an improved safety climate within our department.
