Ventricular Tachycardia Arising from Cardiac Crux

It is seen in patients without structural heart disease and can present as rapid hemodynamically unstable VT leading to cardiac arrest. 12 lead ECG showing RBBB with Q waves in inferior leads, precordial MDI>0.55 and R<S in V5 and V6 localized the VT to apical crux. The VT was successfully ablated in the epicardial cardiac crux through percutaneous pericardial access.
