Better Together: Creating a Longitudinal Interprofessional Team-Based Curriculum (Sarah Coles MD)

Fragmented education within a single professional silo is no longer appropriate to meet the needs of graduating medical students. Interprofessional collaboration, continuity of care, and a focus on population and social determinants of health are the foundation of patient-centered, value-driven care. To meet this need, the Community Health Mentor Program (CHMP) and Longitudinal Patient Care Course (LPC) were designed in collaboration between the University of Arizona College of Medicine in Phoenix, Northern Arizona University College of Health and Human Services, and Arizona State University. Students from schools of medicine, physician assistant, occupational therapy, physical therapy, and nutrition are represented on the teams. The mission of the program is to train healthcare professionals to work collaboratively to improve the quality of care for patients, their families, and their communities. Students are placed into interprofessional teams and paired with a community health mentor, an adult volunteer in the community with a chronic medical condition. Student teams meet with their mentor in the mentor's home every 6 weeks throughout the first year of the course to learn how healthcare values, goals, and medical needs evolve over time, including the effect of the disease process, changing healthcare settings, and the influence of environment and barriers to care.
