Make the Diagnosis: Does This Lesion Need Biopsy?

(MedPage Today) -- A Caucasian man, age 24, with minimal medical history presented for a physical. One of his concerns was this patch of skin on his upper back. He says it has been present in some form for at least 10 years, but he thinks it has gotten slightly larger. It does not itch or hurt. He reports a lot of sun exposure when he was younger, getting dark tans in the summer. However, he has never had any skin lesions removed, nor does he have a family history of skin cancer. He was born and currently lives in Washington state. Is this just an odd cluster of benign nevi or an integrated lesion of concern? Is biopsy required? What is your diagnosis?
