AbstractDetection of residual fibrotic tissue, called ghosts, after lead extraction is a new phenomenon in cardiology. This paper aims at describing the phenomenon of ghosts and determining their characteristic features. The study group consisted of 580 consecutive patients who underwent transvenous lead extraction (TLE) due to local infection, endocarditis and a superfluous lead. THE AngioVac device (AngioDynamics, Vortex Medical Inc, New York, NY) is designed for transcatheter thrombectomy where blood and thrombus material are directly aspirated and then passed through a filter within a veno-venous extracorporeal membrane oxygenation (ECMO) system and returned to the body. Initially approved in 2009 for the removal of fresh thrombus, the AngioVac device has subsequently been used to remove infected casts off pacemaker leads, endocarditis vegetations from the tricuspid valve, and thrombus from the inferior vena cava (IVC), superior vena cava (SVC), the right atrium, and even from the pulmonary arteries.

We report a case of a 59-year-old female patient with vegetative native mitral valve endocarditis caused by Stenotrophomonas maltophilia (SM). She had hemodialysis-dependent chronic renal failure, but no immunosuppressive disease. Echocardiography showed mobile vegetation on her native mitral valve.
