Determination and Characterization of Tetraspanin-Associated Phosphoinositide-4 Kinases in Primary and Neoplastic Liver Cells

Accumulating evidence implicates phosphoinositide 4-phosphate as a regulatory molecule in its own right recruiting specific effector proteins to cellular membranes. Here, we describe biochemical and immunocytochemical methods to evaluate tetraspanin-associated phosphoinositide-4 kinases activity in primary human hepatic stellate cells (hHSC) and neoplastic hepatoblastoma cells.

The epithelial cell adhesion molecule (EpCAM) is involved in oncogenesis of hepatoblastomas (HBs). Prior genomic profiling studies showed higher EpCAM expression and worse prognosis in HBs containing primitive histotypes, however, this has not been fully addressed from an immunohistochemical perspective. Despite recent advances, the prognosis for advanced stage hepatoblastoma remains poor, resulting in a desperate need for novel therapies. We have previously shown that Proviral Integration site for Moloney murine leukemia virus (PIM) kinases act as oncoproteins to promote cell survival in hepatoblastoma. In addition, PIM kinases promote the hepatoblastoma cancer stem cell phenotype, which may contribute to chemoresistance. We hypothesized that a novel PIM inhibitor, PIM447, would decrease oncogenicity and cancer cell stemness in a human hepatoblastoma, patient-derived xenograft (PDX) model.

Children with recurrent hepatoblastoma have a poor prognosis and limited treatment options. The authors describe a child with metastatic hepatoblastoma who had multiple relapses. He was treated with surgical resection and adjuvant chemotherapy at first relapse. At second relapse, he had progressive disease postresection. He was treated with ifosfamide/carboplatin/etoposide chemotherapy followed by maintenance irinotecan and achieved long-term disease-free survival. Irinotecan as maintenance should be explored in recurrent hepatoblastoma.
