The quest for evidence for proton therapy: the model-based approach and precision medicine

Proton therapy can reduce normal tissue dose, but clinical benefit is still insufficiently demonstrated. Patients for whom there is no dosimetric difference in normal tissues of protons compared with photons will not benefit from proton therapy and should therefore neither undergo this therapy nor be included in clinical proton trials. Patient selection for prospective cohort studies as well as for randomized trials should be based on predictable benefit using multivariable NTCP models.
