A Promoter Polymorphism of the Vitamin D Metabolism Gene Cyp24a1 is Associated with Severe Atopic Dermatitis in Adults. Acta Derm Venereol. 2015 Aug 28; Authors: Hallau J, Hamann L, Schumann RR, Worm M, Heine G Abstract Atopic dermatitis (AD) is a chronic inflammatory skin disease in which genetic and environmental factors result in impaired epidermal barrier functioning and an altered immune response. Vitamin D influences these 2 pathomechanisms, and beneficial functions have been suggested in AD. The aim of this study was to investigate the potential roles of the 2 essential vitamin D metabolizing enzymes. The frequencies of 6 common polymorphisms in the genes encoding the vitamin D synthesizing enzyme Cyp27b1 or the inactivating enzyme Cyp24a1 were assessed in 281 patients with AD and 278 healthy donors in a case-control setting. The Cyp24a1 rs2248359-major C allele was significantly over-represented in patients with AD compared with controls, which was more pronounced in patients with severe AD. In addition, haplotypes of the Cyp24a1 and Cyp27b1 genes were associated with AD. These data support that vitamin D mediates beneficial functions in AD and suggest that future studies on the impact of vitamin D on AD should consider the individual genotypes of the vitamin D metabolizing enzymes. However, to do so is impossible in the field of allergy. As allergist-immunologists, we quickly came to understand that allergic diseases are immunologic, and by definition, in volve the entire body. It is true that many allergic diseases manifest predominantly in 1 organ, such as asthma involving the lung or atopic dermatitis involving the skin, yet all allergic diseases are systemic.

Conclusions and implicationsSome preliminary evidence supports the use of zinc in the treatment of acne vulgaris and hidradenitis suppurativa; however, more research is needed with similar methodologies and larger sample sizes in these diseases. Further, zinc may be of some benefit in the treatment plan for atopic dermatitis and diaper dermatitis; however, additional studies should be conducted to further evaluate these potentially positive associations. To date, no evidence is available to suggest that zinc may be of benefit in rosacea and psoriasis; however, limited data are available evaluating the use of zinc in these conditions.
