This study was conducted in the ED of an academic teaching hospital in Miami, Florida. It consisted of a short in-service educational intervention on how to perform and interpret POCUS, followed by an open, prospective convenience sample study in patients with clinical suspicion of shoulder dislocation. Twenty EM residents, with no prior shoulder scanning training, participated in the study. We present a case of medial clavicular physeal fracture mimicking sternoclavicular dislocation diagnosed by ultrasound (US) in a neonate. The infant presented to our clinic at 12 days old with improving left upper extremity pseudoparalysis and outside radiographs interpreted as left sternoclavicular dislocation. US demonstrated a displaced physeal fracture rather than a dislocation. The radiologist should be aware of this potential distinction. ConclusionsThe combination of inferior cortical acromial sclerosis, lateral acromial spur, superior migration of the humeral head, and greater tubercle cysts has a high positive predictive value for the presence of full-thickness rotator cuff tears. In patients with a high suspicion for having a rotator cuff tear based on radiographic findings, MRI can be performed directly without the delay and costs caused by an additional ultrasound exam.
The most common imaging method used to diagnose and monitor bone fractures and healing is multiple radiographic images performed by highly trained professionals with expensive equipment that can expose patients to high levels of ionizing radiation. Here we introduce and illustrate proof-of-concept of a potential alternative method for measuring bone fractures: ultrasound pitch –catch measurement technique. Measurements are performed with two piezoelectric transducers, housed in standard orthopedic screws and fixed on either side of simulated fractures, with and without an orthopedic plate, in ex vivo pig limb bones.
Endotracheal intubation is a commonly performed procedure within the healthcare system. Options for placement confirmation include direct visualization of the tube pass through the cords, fogging in the tube, carbon dioxide colorimetry, end tidal Co2, breath sounds, radiography and bronchoscopy. Breath sounds are often used as the first confirmatory method but are unreliable especially in obese patients. The goal of this study was to evaluate an alternative bedside test that could be used to quickly confirm tube placement in this patient population.
CONCLUSION: Ultrasound as a modality that is easily accessible, inexpensive and without radiation exposure is helpful to demonstrate factors related to pain in knee osteoarthritis by allowing assessment of soft tissue structures. PMID: 31771037 [PubMed - as supplied by publisher]
Radiographs can be used to easily visualize common types of urinary calculi in all parts of the urinary tract. Positive-contrast excretory urography and cystourethrography are sensitive to diseases within the ureters and urethra, most commonly obstruction. Ultrasound is widely available and noninvasive and can be used to evaluate renal architecture, ureteral dilation, urinary bladder wall disease, and urolithiasis. Computed tomography is increasing in availability and provides a large amount of cross-sectional information quickly and noninvasively. Multiple imaging modalities can be used to estimate or quantify glomerular filtration rate.
