NICE are recruiting experts to revise their Fertility guideline

NICE are currently seeking to recruit topic experts for the update of the Fertility (assessment and treatment for people with fertility problems) guideline. The update will look at the effectiveness of intrauterine insemination (IUI), compared with expectant management, in people with unexplained infertility, mild endometriosis or ‘mild’ male factor infertility. They are looking for the following people: 1 x Consultant Level Clinical Scientist (Andrologist/Embryologist) 1 x GP 1 x Fertility Nurse 1 x Lay Member 4 x NHS Fertility Consultants Adverts for both the professional and lay members can be found on the NICE website at the following links: Healthcare Professional Members Lay Members Both adverts will close 5pm on Monday 7th September 2015. Key dates for meetings that must be attended are included in the advert – please take these into consideration when applying

In this study, vaginal microbiome was divided in five biotypes, being four of them dominated by lactobacilli and considered as healthy (pH 5.0) and higher diversity. In a healthy status, microbiota is balanced and forms a stable ecological unit dominated by Lactobacillus species, which fixes pH below 5.0 and controls non-desirable groups. In a pathogenic process, the microbiome shifts to a dysbiotic state, in which lactobacilli drop, pH is increased to values above 4.5 and other groups such as Gardnerella, Atopobium, Prevotella, and Streptococcus, among others, can overgrow (Srinivasan et al., 2012). Short chain fatty acid...

This study evaluated the association between BRCA mutation status and serum anti-Müllerian hormone (AMH) level in young, reproductive-aged patients with breast cancer. Materials and Methods: Patients ≤ 40 years of age with breast cancer and who had known BRCA status and baseline serum AMH level at Samsung Medical Center, Seoul, Korea, were considered for inclusion. A total of 52 BRCA mutation carriers (27 BRCA1 and 25 BRCA2) and 264 non-carriers were selected for analyses. The serum level of AMH was compared according to presence of a BRCA mutation, and linear and logistic regression analyses were performed to eva...

ConclusionsWe have identified menstrual cycle- and endometriosis-associated protein changes linked to various cellular processes that are potential biomarkers and that provide insight into the biology of endometriosis. Our data indicate that the markers tested, whilst not useful alone, have improved diagnostic accuracy when used in combination and demonstrate menstrual cycle specificity. Tissue heterogeneity and blood contamination is likely to have hindered biomarker discovery, whilst a small sample size precludes accurate determination of performance by cycle phase. Independent validation of these biomarker panels in a l...

Authors: Juanqing L, Hailan Y, Xiangwei F, Tao Z, Junyan M, Jianhong Z, Jun L, Jianhua Y Abstract The purpose of this study was to investigate the difference of Twist gene promoter methylation among ovarian ectopic endometrium, eutopic endometrium and non-endometriosis (EMs) endometrium . 15 patients with reproductive age hospitalized at Department of obstetrics and gynecology affiliated to Medicine School of Zhejiang University from January 2013 to June 2016 were selected. Among them, 5 patients underwent laparoscopic surgery due to ovarian type EMs, and were selected after histologic confirmation. Ectopic endomet...

Conclusions and Relevance Cystectomy of ovarian endometriomas has been the first-line treatment for management for many years because it provides improved pain relief, reduces recurrence rates, and was thought to be favorable in in vitro fertilization. However, a growing body of evidence is demonstrating benefit, or at least no harm, in expectant management for asymptomatic patients with small, stable endometriomas. Medical management is often very effective and appropriate first line. When surgical intervention is appropriate, careful ovarian cyst excision with goal of ovarian tissue preservation and treatment of additi...

CONCLUSIONS: The EAOC patients were often diagnosed at a younger age, an earlier stage, and related to nulliparity and infertility. Patients with EAOC had a better prognosis than non-EAOC, early stage rather than association with endometriosis may be the driver of survival. PMID: 30913953 [PubMed - as supplied by publisher]

March is Endometriosis Awareness Month.  Endometriosis is a painful condition where endometrial tissue grows outside the uterus. It is estimated that up to 10% of women have endometriosis (Ozkan 2008).   Endometriosis can cause infertility and for women with subfertility the prevalence rate ranges from 25% to 40% (Ozkan 2008). Endometriosis frequently presents with the symptom of pain (Barlow 1993) including dysmenorrhoea (painful periods), dyspareunia (pain during sexual intercourse), and pelvic or abdominal pain.  We invited Edgardo Somigliana, MD-PhD (Dept Clinical Sciences and Community Health, Universit...

Genesis, genes and epigenetics of endometriosis-associated infertility, Published online: 18 March 2019; doi:10.1038/s41574-019-0191-9Jae-Wook Jeong and colleagues report that histone deacetylase 3 (HDAC3) is downregulated in the endometrium of women with endometriosis. In addition, they show that the loss of HDAC3 is associated with aberrant hormonal signalling and increased fibrosis in the endometrium and impaired endometrial receptivity.
