Importance of assessing the effect of statins on the function of high- density lipoproteins on coronary plaque.

In conclusion, the best coronary plaque regression index attributed to HDL-cholesterol elevation (PH index) was found in pitavastatin treatment, in comparison with the other 4 statins (atorvastatin, pravastatin, simvastatin, rosuvastatin) investigated in the articles scanned by their search. We describe 2 cases of acute carpal tunnel syndrome due to thrombosed persistent median artery, including a case of thromboangiitis obliterans. These thrombosis might also be due to traumatic causes. No guidelines are currently available to help physicians for the management of carpal tunnel syndrome from thrombosed persistent median artery. Antiplatelet therapy, statin, anticoagulant might be helpful, and surgery has sometimes be reported as effective. PMID: 31040049 [PubMed - as supplied by publisher]

Conclusions: Embolism associated with asymptomatic carotid stenosis shows circadian variation with highest rates 4–6 h before midday. This corresponds with peak circadian incidence of stroke and other vascular complications. These and ASED Study results show that monitoring frequency, duration, and time of day are important in ES detection. Introduction Transcranial Doppler (TCD) detected microembolism in the ipsilateral middle cerebral artery (MCA) may help stratify the risk of stroke and other arterial disease complications in persons with advanced (≥60%) asymptomatic carotid stenosis. If so, this techniqu...

In conclusion, the results of the present study indicate that LFU and microbubbles combined with simvastatin promotes the apoptosis of MCF‑7 cells via the LATS1/YAP/RHAMM pathway. The present study suggested a possible strategy for the treatment of breast cancer. PMID: 30015955 [PubMed - as supplied by publisher]

Abstract BACKGROUND: Asian patients with acute coronary syndrome (ACS) are frequently prescribed moderate-intensity statin in real practice, even during the early stage of ACS. Under assessment herein was the effect of moderate-intensity statin therapy on the resolution of plaque inflammation during the first month after ACS, a period with highest recurrent ischemic events, using dual time point ¹⁸F-fluorodeoxyglucose positron emission tomography/computed tomography (FDG PET/CT). METHODS: This prospective study included statin-naïve patients with ACS and non-calcified carotid plaques (³ 3 mm ...

ObjectiveTo assess the effect of tumor necrosis factor inhibitors (TNFi) on subclinical cardiovascular disease in patients with psoriatic disease. MethodsWe performed a 2‐stage study. In stage 1, carotid total plaque area was assessed in patients with psoriasis or psoriatic arthritis (PsA) (n = 319) by ultrasound at baseline and after 2–3 years. The annual progression rate of atherosclerosis was the outcome of interest. In stage 2, PsA patients receiving TNFi (n = 21) and age‐ and sex‐matched PsA patients not receiving any biologic agent (n = 13) underwent 18F‐fluorodeoxyglucose–positron emission tomogr...

We described a 59-year-old man who presented to emergency department complaining of chest pain.Diagnoses:He was suffered acute myocardial infarction (MI) and pulmonary embolism (PE) simultaneously.Interventions:Dual antiplatelet therapy, statin, and low molecular weight heparin were administrated during his stay. The searches for cancers, autoimmune diseases, and hematologic diseases were unremarkable, ruling out a hypercoagulable state. Subsequent ultrasound scan revealed a thrombus in a vein of the lower left extremity. Thus, paradoxical embolism was highly suspected.Outcomes:Paradoxical embolism is a rare cause of acute...

Abstract ObjectivesWe performed a two‐stage study to assess the effect of TNF‐α inhibitors (TNFi) on subclinical cardiovascular disease in psoriatic patients. MethodsWe first assessed carotid total plaque area by ultrasound at baseline and after 2‐3 years in patients with psoriasis and psoriatic arthritis (PsA) (n=319). The annual progression rate of atherosclerosis was the outcome of interest. In stage 2 we studied vascular inflammation using FDG PET/CT in PsA patients on TNFi (n=21) and age and sex matched PsA patients not on any biologics (n=13). This sample underwent FDG PET/CT scans at baseline and 1 year ...

ConclusionThe underlying pathophysiology of PCOS is still not fully understood. As a result, treatment is often focused on individual symptoms, not the syndrome itself. Robust laboratory led protocols would provide the necessary information to enable an appropriate diagnostic evaluation/cardometabolic monitoring.

By ANISH KOKA, MD You may have heard that repealing and replacing Obamacare recently failed.  The analysis of what went wrong comes from many corners.  Andy Slavitt, former insurance executive and most recent director of CMS, writes that the ‘failure of Trumpcare can be seen as a rejection of policies that Americans judged would move the country backward.’  Apparently, the theory goes, moderate republicans, especially in states that expanded heavily and rely on Obamacare Medicaid expansion, were skittish of a repeal and replace plan that endangered the healthcare of millions of constituents. &nb...
