Anticoagulation stewardship: Descriptive analysis of a novel approach to appropriate anticoagulant prescription

CONCLUSION: Inpatient anticoagulant prescriptions were optimized following recommendations by the ASP team. The most frequent types of prescription changes concerned dose adjustments and drug interactions.
