ConclusionsThe PTH-based classifier may help with individualised treatment planning for selecting patients who may benefit from thermal ablation.Key Points•This is the first analysis of predictors affecting the outcome of US-guided thermal ablation of primary hyperparathyroidism and the findings can be used to identify the potential beneficiary population of thermal ablation of primary hyperparathyroidism.•Parathyroid hormone (PTH) was confirmed as an independent prognostic factor, as it not only showed good accuracy in stratifying patients into high- and low-risk groups in the training and validation cohorts but also outperformed the clinical model.•This study developed and validated a model to predict the treatment success of thermal ablation of primary hyperparathyroidism.
