Short-term effects of small volume saline infusion on acid base equilibrium in critically ill patients
CONCLUSIONS: The infusion of 0.9% saline reduced strong ion difference by increasing plasma chloride concentration, with no effect on pH due to concomitant albumin dilution. Saline non-responders, characterized by the ability to excrete the sodium excess, were more likely to suffer the acidifying effects of saline infusion, while renal function did not affect the acid-base response to saline infusion.PMID:38652451 | DOI:10.23736/S0375-9393.23.17784-4 (Source: Minerva Anestesiologica)

Lumbar intervertebral disc degeneration in low back pain
Minerva Anestesiol. 2024 Apr;90(4):330-338. doi: 10.23736/S0375-9393.24.17843-1.ABSTRACTIntervertebral disc degeneration is characterized by deterioration in structural support that is potentially followed by stimulated neuronal ingrowth, and dysfunction of cellular physiology in the disc. Discogenic low back pain originates from nociceptors within the intervertebral disc or the cartilage endplate. This narrative review examines the mechanisms of disc degeneration, the association between degeneration and pain, and the current diagnosis and treatment of discogenic low back pain. Mechanisms of disc degeneration include dysr...

Clinical prediction of opioid use disorder in chronic pain patients: a cohort-retrospective study with a pharmacogenetic approach
CONCLUSIONS: Functional polymorphisms related to OPRM1 variant and CYP2D6 phenotypes may predict a higher OUD risk. Established risk factors such as young age, elevated MEDD and lower incomes were identified. A predictive model is expected to be implemented in clinical setting among CNCP patients under long-term opioids use.PMID:38619184 | DOI:10.23736/S0375-9393.24.17864-9 (Source: Minerva Anestesiologica)
