In Reply We thank Fu and colleagues for the opportunity to clarify the design of our study, preventing a misreading that is critical to the interpretation of the work. Fu et al raised concern that imperfect adherence to prescribed thyroid hormone may have led to an overestimate of “the risks associated with prescribed doses of exogenous thyroid hormone.” However, the exposure in our study was not prescribed thyroid hormone, but rather thyrotoxicosis as measured by thyrotropin laboratory values. Exploring the Link Between Thyrotoxicosis and Cognitive Decline in Older Adults
To the Editor In the pivotal cohort study by Adams et al, an in-depth evaluation of the association between thyrotoxicosis and cognitive decline in older individuals was conducted. Analyzing data from 65  931 individuals 65 years and older within the Johns Hopkins Community Physicians Network (2014-2023), the study discerns a critical association between reduced thyrotropin levels and elevated occurrences of cognitive impairments, including dementia.
